Health Access Navigation Services (HANS)

Eligibility

Catchment criteria:

Must be a resident of Parkdale area (Bloor Street West to Lakeshore Blvd West; Parkside Dr to Dovercourt Rd) AND must be experiencing any of the following below:

  • Frequent hospital Emergency Department visits
  • A minimum of 3 admission to hospital in one year for a chronic health issue
  • Individuals with complex health needs
  • One recent/current acute hospital admission
  • Have active health care goals.

The Health Access Navigation Services (HANS) is a time-limited support focused on system navigation and care coordination that supports clients to transition back into the community and reduce hospital readmissions. Successful referrals will receive timely access to community supports, care coordination between new and existing services, and discharge planning once service integration has been secured.

Services/Supplies Offered

  • Short-term case management (up to 3 months)
  • Referral and connection to holistic and clinical services
  • Facilitate the development or integration of a comprehensive care team.
  • Oversee care planning and care coordination.
  • Accompaniments and home visits with/to clients to appointments
  • Discharge planning.
