The Mid West Toronto HART (Homeless and Addiction Recovery Treatment) HUB offers wraparound care for community members facing systemic barriers – including those navigating challenges with housing stability, substance use, mental wellness, and access to social services. Across 11 partner organizations, we are able to offer:

• Primary care & substance use treatment
• Mental health assessments & referrals
• Housing & income stabilization support
• Case management and system navigation
• Support in accessing social assistance programs and services
• Culturally safe care, including access to Indigenous healing practices

Please note: Supervised Consumption Services (SCS), safer supply programs, drug checking, and sterile needle distribution are not available at 168 Bathurst Street.
