The HART (Homeless and Addiction Recovery Treatment) HUB offers wraparound care for community members facing systemic barriers – including those navigating challenges with housing stability, substance use, mental wellness and access to social services.
Led by an interprofessional team of physicians, nurse practitioners, mental health professionals, housing support workers, case managers, and peer advocates, the Hub will provide:
- Primary care & substance use treatment
- Mental health assessments & referrals
- Housing & income stabilization support
- Case management and system navigation
- Support in accessing social assistance programs and services
- Culturally safe care, including access to indigenous healing practice
Please note: Supervised Consumption Services (SCS), safer supply programs, drug checking and sterile needle distribution are not available at 168 Bathurst Street.
