On November 18, the partner organizations of the Mid-West Toronto region have been approved to become an Ontario Health Team, marking a significant milestone for patients, clients, families and caregivers in downtown Toronto’s west-end.
Parkdale Queen West Community Health Centre is proud to be one of more than 50 health care and community service providers that have collaborated to form this OHT. The team is also proud to partner with over 250 primary care providers in our region. Together, over the last 18 months, we have collaborated to begin to reimagine how care will be provided to the people we serve.
