Parkdale Queen West Community Health Centre (PQWCHC) is a community-based health service organization in southwest Toronto. We provide essential health services such as primary healthcare, dental care, harm reduction education, counselling, mental health support, health promotion, wellness group programming, and more.
Born from the former Parkdale and Queen West Community Health Centres merger in 2017 and governed by a Board of Directors, it is an accredited community health centre and a member of the Alliance for Healthier Communities.
Our focus is on individuals and communities facing barriers to quality healthcare, including vulnerable groups like newcomers, seniors, Indigenous and racialized communities, LGBTQ communities, homeless or street-involved individuals, people who use drugs, low-income people, and people living with mental health conditions, amongst others.
Parkdale Queen West CHC is a non-profit registered charity, operating primarily from four locations: 168 Bathurst Street, 1229 Queen Street West, 27 Roncesvalles Avenue, and 750 Dundas Street West. We also deliver care at outreach locations where it’s needed most, such as shelters, partner organizations, and community events, to ensure accessibility for all.

Parkdale Queen West Community Health Centre is the result of a voluntary merger of the former Parkdale and Queen West Community Health Centres, effective on April 1, 2017.
We work to improve the health and well-being of individuals and communities at risk and/or facing barriers to accessing high quality healthcare services and supports. Priority is given to individuals and groups who encounter obstacles to high-quality healthcare services, including marginalized or vulnerable populations such as newcomers, Indigenous and racialized communities, low income people, people who are homeless or street-involved, people living with mental health conditions, people who use drugs, isolated seniors, and LGBTQ communities.
Our services are primarily offered at each of our four locations: 168 Bathurst Street, 1229 Queen Street West, 27 Roncesvalles Avenue, and 750 Dundas Street West. We also provide primary care services and deliver harm reduction information, education and supplies at several outreach locations within our catchment, including street outreach locations where homeless people occupy, rooming houses, drop-in centres, shelters, partner organizations and community events.
