Getting and replacing identification is difficult for those on the street or in the shelter system. The Identification Service helps people obtain an Ontario Health Card and, when necessary, apply for a birth certificate or proof of legal status. These documents then support an application for a social insurance number.
In a partnership with Street Health, we provide identification clinics for individuals looking to replace or obtain legal Canadian identifications, including:
- Ontario Health Card
- Social Insurance Number
- Canadian Birth Certificates (all provinces and territories)
- Verification of Status or Record of Landing for clients born outside of Canada (ROL)
- Ontario Statement on Live Birth (SOLB)
- Ontario Delayed Statement on Live Birth (D SOLB)
- Ontario Photo ID
- Canadian Marriage Certificate (MC)
- Canadian Death Certificate (CDC)
Please allow three to six (3-6) weeks for delivery.
The ID Team doesn’t assist with:
- Passport
- Driver’s License
- Permanent residence card
- Citizenship Card
