Overview

Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a community-based health service organization located in south-west Toronto. We offer a broad range of services, including primary health care, dental care, harm reduction, health promotion, counselling, and community development programming. We offer services at three main clinic locations, as well as within the community.

Parkdale Queen West Community Health Centre (Parkdale Queen West CHC) is a non-profit registered charity, community-based health and wellness service organization, governed by a Board of Directors. We are an accredited Community Health Centre and a member of the Association of Ontario Health Centres.

Parkdale Queen West Community Health Centre is the result of a voluntary merger of the former Parkdale and Queen West Community Health Centres, effective on April 1, 2017.

We offer a broad range of services including primary health care, dental care, health promotion, counselling and mental health supports, HIV and Hepatitis C supports and education, anonymous HIV point of care testing, wellness group programming, practical supports (legal, employment and housing), harm reduction and illness prevention, advocacy, and community engagement and development.

We work to improve the health and wellbeing of individuals and communities who are at risk and/or face barriers to accessing high quality health care services and supports. Priority is given to individuals and groups who encounter barriers to high quality health care services, including marginalized or vulnerable populations such as newcomers, Indigenous and racialized communities, people who are homeless or street-involved, people living with mental health issues, people who use drugs, people living with disabilities, isolated seniors, LGBTQ communities and people living in poverty.

Our services are primarily offered at each of our three locations, being 168 Bathurst Street, 1229 Queen Street West, and 27 Roncesvalles Avenue. However, we provide primary care services and deliver harm reduction information, education and supplies at a number of outreach locations within our catchment, including street outreach locations where homeless people occupy, rooming houses, drop-in centres, shelters, partner organizations and at community events.
