PQWCHC supports the multiple vaccination efforts taking place across the City of Toronto.   If you need help with booking a vaccine appointment, please call 416-703-8482, ext. 2234.

PQWCHC is able to support the community in downloading and printing vaccine receipts as needed. For support with this, please call

Deb (Queen West site) at ext. 2234 or Sally (Parkdale site) at ext. 1265. We will make every effort to accommodate walk in requests for vaccine receipt printing at both main locations.

To book your first or second dose:

First Doses:
People aged 12+ in the province of Ontario are eligible for their first dose.

Second Doses
Eligibility for second doses has been accelerated.

If you received Astra Zeneca as your first dose, the current guidance is to receive your second dose 12 weeks later.  You will have the choice of Astra Zeneca, Pfizer, or Moderna as your second dose.

Who is eligible (Pfizer or Moderna as first dose)

May 31

·       People aged 80 and over

·       People aged 70 and over (in pharmacies or primary care settings only)

·       People who received their first dose on or before April 18 (in pharmacies or primary care settings only)

June 7

·       People aged 70 and over (through the provincial booking system)

·       People who received their first dose on or before April 18 (through the provincial booking system)

June 14

·       People in who received their first dose on or before May 9 and who live in Halton, Peel, Porcupine, Toronto, Waterloo, Wellington-Dufferin-Guelph or York public health units

July 19

·       People who received their first dose on or before May 9

August 2

·       People who received their first dose on or before May 30

August 9 and 16

·       People aged 12 to 25

August 9

·       People who received their first dose May 31 onwards (respecting the recommended intervals)
